A 64-year-old woman presents with left hemiparesis, left face and arm clonic seizures, and worsening encephalopathy for 3 days. Neurologic examination demonstrates expressive aphasia, midline gaze with full extraocular movements, reduced muscle strength in the left arm and leg (grade 4 out of 5), and left-sided neglect.
